Consent of a Prescribed Body for its Particulars to be Printed on a Ballot Paper in Relation to the Request by Candidate(s) [REO/BP/9/2004LC(SF)]

Intention to Display Election Advertisements at Designated Spots [REO/C/23(FC)/2004LC]

The following forms should be completed and submitted to the Returning Officer at the time of the submission of nomination form or before the close of the nomination period (4 August 2004) -

-	Completed grid paper (an original and a photocopy) ;

-	Request by a Candidate for Functional Constituency or a Sole Candidate on a Nomination List for Printing of Particulars Relating to the Candidate on a Ballot Paper [REO/BP/7/2004LC(SF)] (if applicable) ;

-	Request by Candidates on Nomination List with More than One Candidate for Printing of Particulars Relating to the Candidates on a Ballot Paper [REO/BP/8/2004LC(SF)] (if applicable) ;

-	Consent of a Prescribed Body for its Particulars to be Printed on a Ballot Paper in Relation to the Request by Candidate(s) [REO/BP/9/2004LC(SF)] (if applicable) ; and

-	Intention to Display Election Advertisements at Designated Spots [REO/C/23(FC)/2004LC] or [REO/C/23(GC)/2004LC].

The above-mentioned forms may be obtained from District Offices, the office of the relevant Returning Officer or from the Registration and Electoral Office, 10th floor, Harbour Centre, 25 Harbour Road, Wan Chai, Hong Kong.
